The Local Homeless Coordinating Board (LHCB) is the lead entity for the San Francisco Continuum of Care. It works to ensure a unified homeless strategy that is supported by the Mayor, the Board of Supervisors, City departments, nonprofit agencies, people who are homeless or formerly homeless and the community at large. The LHCB serves as an advisory body to the Department of Homelessness and Supportive Housing. All efforts are aimed at permanent solutions, and the range of services is designed to meet the unique and complex needs of individuals who are threatened with or currently experiencing homelessness. HSH is an active participant in the LHCB Coordinated Entry Committee meetings.

Youth Coordinated Entry Access Points
Coordinated Entry Access Points provide housing problem solving, assessment, prioritization, and referral to shelter, housing, and other community services for young adults in San Francisco experiencing homelessness. Young adults between 18 and 27 who are experiencing or at risk of homelessness can contact the Access Points listed below. Some housing and shelter services are only available for people under 25.

Coordinated Entry Access Points provide housing problem solving, assessment, prioritization, and referral to shelter, housing, and other community services for San Francisco families experiencing homelessness. Families with minor children who are experiencing or at risk of homelessness can contact the Access Points listed below.
If Access Point staff are busy communicating with other community members, please leave a voicemail with your name, contact information and preferred language. Your call will be returned within one business day. Mobile response teams are available to meet families anywhere in the city who are unable to visit an Access Point.
