Relocation Assistance Programs

San Francisco offers three programs that help people who are experiencing homelessness or at risk of homelessness reunite with support networks and stable homes outside the city. We track how many people we’ve served in each program in the dashboard below.

  • Problem Solving Relocation Assistance: Offered by the Department of Homelessness and Supportive Housing (HSH) for unhoused clients.
    • This program started in July 2022 as the successor to the longstanding Homeward Bound program.
  • County Adult Assistance Program (CAAP) Relocation Assistance: Offered by the Human Services Agency (HSA) for CAAP recipients.
    • This program started in April 2023.
  • Journey Home: Offered by HSA and HSH as part of the Drug Market Agency Coordination Center (DMACC) for unhoused people and people with substance use disorders.
    • This program started in September 2023.

Relocation Assistance Programs

These three programs all help people relocate outside San Francisco to reunite with family or support networks. Each program has different entry points and slightly different resources.

HSH: Problem Solving Relocation Assistance

For:People experiencing homelessness (including people about to lose their housing) with income up to 50% of the area median income (AMI).
Services:Travel assistance, coordination with support networks, and flexible financial assistance for moving into housing in new community. 
Access:HSH Access Points, family shelters, and five community locations.
History:Fully launched in July 2022 as one element of HSH’s problem solving services for unhoused people.
This service replaced a previous program called Homeward Bound that only offered bus tickets. Homeward Bound was operated by HSA from 2005 to 2016.HSH operated the program from 2016 to 2022, when it transitioned into problem solving. Between 2005 and 2022, Homeward Bound helped households relocate 10,600 times.

HSA: County Adult Assistance Program (CAAP) Relocation Assistance

For:County Adult Assistance Program (CAAP) clients, housed or unhoused. Includes clients without an active case (i.e., in application phase or discontinued).
Services:Travel assistance and coordination with support networks.
Access:CAAP office at 1235 Mission Street. 
History:Launched in April 2023 to expand on HSH’s relocation services. 

HSA & HSH: Journey Home

For:People experiencing homelessness and/or people with substance use disorders.
Services:Travel assistance and temporary shelter (as needed). Lowest-barrier approach to quickly help people in need relocate. 
Access:Referrals from outreach teams, law enforcement, and any other group or individual self-referrals. Call (669) 265-9373 from Monday to Friday, 8AM – 5PM. Teams respond in the field.
History:Launched in September 2023 as part of the Drug Market Agency Coordination Center (DMACC).  

Housing Problem Solving

Housing problem solving includes interventions to divert or rapidly exit people from homelessness.

Problem solving helps people identify possible pathways to resolve their current housing crisis without needing ongoing shelter or a housing resource from the homelessness response system (HRS). The foundation of problem solving is a creative, strengths-based conversation that helps people explore all safe housing options available to them – the person or household drives their own solutions. A problem solving resolution is achieved when a household has found a safe, indoor solution to their housing crisis outside of the HRS.  

We publish monthly updates about the number of households receiving problem solving assistance. We also report on people served through the City’s relocation assistance programs.

Eligibility

Any household experiencing homelessness in San Francisco is eligible for problem solving Interventions (refer to HSH’s definition of homelessness document) who has an annual household income at the time of assistance no higher than 50% of the Area Median Income (AMI).

Interventions

Housing problem solving interventions include:

  • Problem solving conversations: help identify real-time solutions to a housing crisis
  • Housing location assistance: helps households with income but without an immediate housing plan locate a place to rent
    • Includes shared housing placements to increase exits to housing
  • Travel and relocation support outside San Francisco: travel and relocation assistance that results in a housing connection/safe housing plan in another community
    • The Human Services Agency leads two additional relocation assistance programs: Homeward Bound and Journey Home. Get more information on our Housing and Problem Solving Assistance page.

  • Reunification, mediation, and conflict resolution: helps households stay in a current or recent housing situation or new housing situation with mediation support
  • Financial assistance: Flexible financial resources to cover specific costs that will assist households to stay in a safe, indoor place outside the HRS
  • Connections to employment: currently a pilot with OEWD
  • Referrals and links to a range of community services

Problem solving is offered at Coordinated Entry Access Points, five community locations, and family shelters.

Coordinated Entry

Coordinated Entry is the “front door” to the homelessness response system (HRS). It is designed to assess, prioritize and match people experiencing homelessness to housing opportunities efficiently and consistently. Coordinated Entry uses a locally designated population-specific assessment, a centralized data system, a “by name” database of clients, and a prioritization method.

Coordinated Entry Access Points are the community gateways into San Francisco’s HRS and serve: adults, families and young adults ages 18 to 27. Some Access Points focus on subpopulations that benefit from a tailored approach, like veterans and justice-involved people. Access points connect clients to housing, housing problem solving, and other resources. Youth and families can access shelter at these sites.

  • Families with children under the age of 18 can use Family Access Points.              
  • Adults over the age of 18 without minor children can use Adult Access Points. 
  • Young adults (ages 18-27) can use the Youth CE Access Points, although some services are only available for people under 24.

All Access Points provide the same assessment approach.

Coordinated Entry Testimonial:

Barry*, a San Francisco native, recently moved into his new home after experiencing homelessness for over two years.  After his divorce, Barry alternated between couch-surfing, staying in shelters, and living on the streets.  He suffered a stroke, which resulted in him having significantly limited mobility and difficulty speaking.  During the COVID-19 pandemic, Barry received a room at a Shelter-in-Place hotel operated by Episcopal Community Services.  With the assistance of Adult Coordinated Entry, he secured permanent supportive housing, and he says he is delighted to “move forward and live my life” now that he has housing stability.

Homelessness Oversight Commission Meetings

The Homelessness Oversight Commission will meet on the first Thursday of every month. Meetings are conducted in-person and will be live-streamed on SFGovTV or Channel 78. Remote access information for virtual public comment is included in each agenda.

San Francisco’s Department of Homelessness and Supportive Housing (HSH) is soliciting Applications from community-based organizations to operate a Flexible Housing Subsidy Pool (FHSP) program for the City-wide Ending Transgender Homelessness Initiative (ETH). ETH is a collaborative partnership providing funding for behavioral health services and housing interventions for TGNC San Franciscans experiencing homelessness. Among these housing interventions is FHSP, a permanent scattered-site subsidy program offering housing location and case management services and capping participant rent payments at 30% of income. HSH seeks providers who will deliver, or collaborative with other providers to deliver, these services to TGNC residents with a trauma-informed and equity-driven approach.

Solicitation of Interest – Problem Solving Services (non-Access Point)

Description

The Department of Homelessness and Supportive Housing (HSH) is soliciting applications from community-based organizations to provide Problem Solving services to households (families, youth and/or single adults) experiencing homelessness, as defined by the San Francisco Department of Homelessness and Supportive Housing (HSH) definitions. 

The Department is seeking qualified applicants who demonstrate the necessary expertise, experience, and capacity to provide Problem Solving Services. The successful delivery of Problem Solving requires staff to truly understand the dynamics of homelessness, be skilled at motivational interviewing techniques, and embrace and embody strength-based, person-centered, trauma-informed, and harm-reducing principles.

Problem Solving services are available to any household experiencing homelessness in San Francisco who has an annual household income at the time of assistance no higher than 50% of the Area Median Income (AMI). Problem Solving services are being offered in all HSH Access Points and at 12 Family Shelters. One of the purposes of this SOI is to be able to reach a broader community (especially households who may never interact with the Homelessness Response System) by selecting a Problem Solving provider who will not function as an Access Point.

The Department of Homelessness and Supportive Housing (HSH) is seeking to fund a youth- serving community partner to design, implement, and manage a direct cash transfer (DCT) program pilot for youth between the ages of 18-24, usually referred to as Transitional Age Youth (TAY). Based on the most recent Point-in-Time count (conducted in 2019), there were 1,145 youth experiencing homelessness on a single night in San Francisco; 95% of these youth were TAY. In an effort to reduce the number of TAY experiencing homelessness in San Francisco, to increase TAY’s housing stability, and to expand the City’s inventory of available interventions to support ending youth homelessness, HSH is interested in piloting a DCT program.

Solicitation of Interest (SOI) Coordinated Entry and Problem Solving Services to the Justice-Involved Population

Description:  

The Department of Homelessness and Supportive Housing (HSH) is soliciting applications from community based organizations to provide Coordinated Entry and Problem Solving services to the justice-involved population. The goal of these services is to bridge the gaps between the justice involved population and the Homelessness Response System (HRS).

HSH is seeking community-based organizations who demonstrate the necessary expertise, experience, and capacity to provide Coordinated Entry and Problem Solving Services to ensure the justice involved population experiencing homelessness has equitable access to the HRS.

After community input and consultation with experts in the justice system, HSH has identified households with individuals who are incarcerated in the San Francisco County Jail and are expected to be released to the community and those who have been granted pretrial release as the targeted population for this funding. This community-based agency will take part in a citywide effort among various city Departments and other community-based agencies to have a more intentional and holistic program delivery to the justice-involved population, starting with this specific sub-group and in future years expanding to other groups.

Description: To assist the City and County of San Francisco with plans for providing permanent supportive housing, the Department of Homelessness and Supportive Housing (“City” or “HSH”) invites interested property owners and authorized representatives to submit information to assist the City in identifying suitable properties for the possible acquisition or master leasing of permanent supportive housing sites to house people currently experiencing homelessness. Request for Potential Shelter and Housing Sites

Description: The City and County of San Francisco’s Department of Homelessness and Supportive Housing (HSH) is collecting basic information directly from property owners or persons/organizations with site control regarding locations for possible consideration as shelter sites. HSH is seeking sites for temporary/short-term (as short as 2 to 7 days) or seasonal use (several months) during periods of severe weather, environmental emergencies or natural disasters, and sites for potential year-round use as emergency shelters.

HSH is particularly interested in leasing or purchasing sites with the following characteristics:

  • located along public transportation lines and/or convenient to neighborhood services
  • existing non-residential buildings that could be converted to residential/dormitory use
  • vacant land on which HSH could construct a shelter

If you have a property that you would like to discuss with HSH, please provide a summary of the site features itemized in the table found on this document. If you have more than one property, please provide a summary for each property. Submission of information is not a commitment on the part of the owner/operator in any form. It is simply a show of interest to explore possibilities.

HSH is accepting this information as soon as possible for the coming fall/winter months and on an ongoing basis. We will keep the property information on file and contact owners if and when there is funding and interest to explore the property.
